Social Worker/Case Manager

SUPERVISES: N/A
JOB SUMMARY Case management including case monitoring, new client intakes, outreach, and coordination with services of other agencies to assist each client in making optimum use of the programs. Serves mainly Hillsborough County.
MAJOR DUTIES Overall case management responsibility of assigned clients including intake, advising, referral, case monitoring, plan revision, and follow-up. Counseling to assure optimal adjustment to visual impairment and benefit from rehabilitation training. Provision of information, referral, and follow up services to TLFTB programs and other community services, as appropriate. Especially referral agencies such as Division of Blind Services. Enter reports-daily in agency and Statewide databases. Participation in all agency case-staffing (including DBS clients). Outreach activities and events to promote Lighthouse programs. Contribute suggestions and participate in activities relating to Social Services, and in some instances, the total agency, e.g., Newsletter, media coverage, etc. Home visitation. coordination and management of weekly case management meetings and monthly meetings with Division of Blind Services and Lighthouse staff. Coordinate and participate in support groups. Client escort or transportation services as necessary. Other duties as assigned.
MINIMUM QUALIFICATIONS: BSW or MSW degree from an accredited school of social work strongly preferred. Must be able to carry out casework on group assignments with a decreasing need for supervision. Must have access to reliable transportation. Ability to communicate in both written and verbal forms. Good computer and data entry skills.
